GRAUATION NAME SURVEY DUE Parents, please complete the GRADUATION NAME SURVEY by November 1, 2025, to indicate student name preferences for graduation yard sign, name read aloud at graduation, name listed in graduation program, and name listed on the student diploma. This survey is crucial to honoring your and your student’s preferences for these various documents. Please note that the high school diploma is a legal document and the name listed on their diploma should be their FULL Legal Name (as it appears on their Birth Certificate). Please include any accents or special characters if it is a part of their Legal Name. If your student plans to change their legal name in the future, you can contact FHS at that time to order a new diploma when that legal change occurs (a reprinting charge will occur). If you do not complete this survey by November 1, we will use the name listed on your student’s birth certificate for all documents.
