Take the Fishers Health Department Community Health & Wellbeing Survey

The Fishers Health Department (FHD) invites residents to participate in the 2025 Community Health & Wellbeing Survey, an essential tool for understanding the health needs, priorities, and challenges of those who live in the city. Open now to all Fishers residents ages 18 and older, the survey is completely anonymous and takes approximately 15–20 minutes to complete. The survey is available in both English and Spanish. 

By participating, residents will help guide decisions that impact local public health services and policies, including mental health support, access to healthcare, nutrition and fitness programs, emergency preparedness, chronic disease prevention, and more.  

“Public health starts with listening,” said Monica Heltz, Fishers Public Health Director. “This survey gives us a chance to hear directly from the people we serve—what’s working, where the gaps are, and how we can do better. We want every voice at the table so we can align our programs and resources with real community priorities.” 

From access to healthcare and mental health services to safe neighborhoods, nutritious food, and reliable transportation, public health touches nearly every aspect of daily life. The survey will help FHD: 

  • Identify urgent community health needs and barriers to care 
  • Improve and expand local services, including mental health, emergency preparedness, and health education 
  • Track the impact of local initiatives
